Yes, your planeswalker would exile itself and return to the battlefield with 5 loyalty counters on it unless he is at 1 loyalty when you activate the ability. In which case he would just go to your graveyard.

@Corwinnn Tested and the same method can be used to get any kind of symbol in a planeswalker's ability box. Though a similar problem arises in that case.
