Not even close to balanced! That's also a horrible design practice in terms of how to buff existing cards. Keep in mind penalties much be larger than you'd be willing to have in order for balance.
Edit: "Each opponent may scry 3." would work though if you're really set on the idea.

Players don't have a maximum life total. Thus your card doesn't work and need changes.

Well, my thoughts were if you casted a Lightning Bolt and sacrificed a Corrosive mentor, you could deal 3 damage in -1/-1 counters, or if you casted a Leyline of sanctity and sacrificed a Darksteel Myr, the enchantment would have Indestructible. Hope this helps to explain
Wait, I just realized: Sorin's Vengeance (or other damage spell.) while sacrificing a creature with infect...

I made this to represent Razaketh, Liliana's demon on Amonkhet. I assume we'll be getting his proper card during Hour of Devastation. Apparently he's more powerful than Grislebrand. So how did I do?
Is Legacy too easy to break even with high costs?
Would anyone actually try this card in anything but sealed
