This one I can illustrate with a card that fetches the planeswalker in question (like Liberating Combustion fetches Chandra, Pyrogenius).
Vardrun was originally an incubus of the Blackmist cadre that had chosen to expand his repertoire by dabbling into emotional magic and mastering the Blackmist to let him avoid danger during his travels. While he was resting between journeys at the Fortress of Dargar, an unforeseen manasurge created a blizzard of terrifying proportions. The defenders, caught unprepared, were ready to give up. If it hadn't been for Vardrun rallying their forces to contain the blizzard and buy the clerics enough time to cleanse it, the entire fortress would've been swept away.
It was during the fight, pouring his magic into the containment field, that he strained his soul to the limit and his spark ignited. When he returned from his planeswalk, he feared the worst, and was relieved to find the fortress still standing. Since then, he's remained a guardian of the plane, roaming the land and inspiring others to fight its wild dangers.

Quite handily, here is a link to my set, which contains the cards I've created for the plane of Nittaalaq.
Nittaalaq is a plane that has no magma and a distant sun, so it is always cold. Instead of lava, the world's core is made of liquid mana of all colours. This mana is highly unstable and tends to build up across the plane's leylines and places of power. When the buildup becomes too great, it produces a manasurge, which takes the form of some sort of supernatural disaster, or even sometimes takes the shape of wild, terrible creatures.
The races of Nittaalaq have long cooperated to keep these manasurges in check, with each doing its part to keep the plane safe. Red-aligned races inspire, they give others the courage and will to fight, and are the first to spring into action. Black-aligned races provide, they give others the power they need to accomplish their goals, and are the intermediaries between the rest. Blue-aligned races contain, it is their duty to prevent manasurges and other disasters from spiralling out of control, and it is they that buy everyone else time to do their jobs. White-aligned races cleanse, it is their duty to dispel the terrible things created by the excess mana, and to keep the leylines and places of power safe. Green-aligned races restore, it is their duty to heal and regrow after the disaster has passed, and to ensure that victories are not Pyrrhic.
The plane of Nittaalaq is cold, beautiful and dangerous, encouraging cooperation in the face of overwhelming, unpredictable odds.

Jakob was a summoner in Boltan, and lived happily, when the city was attacked by the Phyrexians. As his friends charged into battle, he summoned elementals to fight with them. He looked on, as his friends were slaughtered, which ignited his spark. As the city swarmed with Phyrexians, he planeswalked away. He vowed to destroy the threat of the Phyrexians and is currently fighting them.
@MrRansom The Planeswalker's +2 is probably too strong. A +1 that makes one spirit might be better.
Sandvad is a mage from the plane of Myrin. Ever since he was a child, he'd dreamt of going on adventures and discovering everything there was to know about Myrin. Eventually, he apprenticed to an unnamed nomadic wizard. While they traveled Myrin, they became good friends. Sandvad had written a book containing everything he had learned, and was to publish it, when the unthinkable happened. The wizard who had mentored him for so many years suddenly died. Stricken with grief, his planeswalker spark ignited, and he landed on Theros. END OF LORE.

LORE:
Rashía was an elf on her home plane of Lakano, when she was transported through a portal to a new plane of Syranor. The elves there were knowledgeable about planeswalkers and helped Rashía to ignite her spark. However, as she has come to Syranor through a portal, she did not know how to return to Lakano through the blind eternities, thus condemning her to wander the planes until she could find a way to return to her home.

That's why I said "re-vamp".
The card was never technically complete. I've been absolutely swamped (just finished my 6th, 2 hr production performance at a church a bunch of my friends attend last night) and I had no time between seeing you upload this and judge it wherein to complete a suitable entry...
Now elves of Llanowar lived in the treetops, and in villages along the forest floor, and some still live in caves and burrows. But a long long (VERY LONG) time ago, there were many elves whom lived under the soils of the forests of Llanowar. They had decided that adapting to life underground was the most cunning and resourceful defense against the powerful neighboring creatures of the forest.
Living underground can be quite boring. Games were invented. Sex positions were invented. New forms of magic were invented. Anything to pass the time.
Llanowar spiritualists and witchdoctors spent a generation or more practicing what later become known as "herbalism" among the elves. They enhanced the natural nutrients and ailment properties of the plants of the forest. They grew super-plants. Saplings that could fight for them. Lotus blooms that had a much greater chance of producing the mana that they craved so much.
But one day, one of the witchdoctors took his powers too far. It all started when he happened upon a new species of chard. He named it Morningslice. And unaltered, it was a vegetable that could heal dire sickness in a matter of hours. A leaf of it could fill an elf's appetite for 3 days. It was the new secret miracle food of the underground Llanowar. But then, the witchdoctor experimented with the plant until it mutated into the titan known as Chard.
And with his wrath, Chard overthrew the leaders of the underground clans of Llanowar Elves. He cast the elves back to the forest floor, where most of them died in a matter of minutes as they had no survival skills since they had been sheltered all of their lives. Only the quietest, stealthiest elves with a will to survive and a resourcefulness that's unmatched were able to carry-on the original clans of underground elves. Their herbalism failures, as well as the creation of Chard, did not deter them from using their skills to make poisons and potions and anything to give them the edge against their enemies. In fact, the fear of dying is a large contributor to what make the Llanowar Elves a swift and fearsome force to be reckoned with; whether in the canopies, on the forest floor, or even underground.
No one knows why Chard was born into evil. Or what gave him the right to take what doesn't belong to him. But the elves will bide their time until they figure out a surefire way to take back their long lost ancient home.
