Insanity counters work like this
On that player's upkeep they need to sacrifice permanents equal to the amount of insanity counters they have or they will be dealt 6 damage

Perla VS. Threok
Threok is a king's slave so he can't fight.

Nyarlethotep starts off with trying to put Teri into Insanity but with Teri being as cool as she is she just shrugs it off. STOP! I have some personal Biases in this, but in all around best design, it's Teri.
