  Creatures can't transform into a planeswalker directly without exiling it. Because loyalty counters are put only when planeswalker enters the battlefield. So if you want to transform a creature into a planeswalker, you need to exile a creature first, then return it transformed.
  
  But I think there's another way. Put a loyalty counter on the creature, then transforms it. But in this case, you need to write 0 in the box of loyalty of the planeswalker.
