Shall we have teams be chosen by team captain? This would be my most favored option, since I know most of the team captains better.
Shall we have multiple rounds? I don't think so, considering the amount of investment one round alone will take. Plus, it doesn't seem like a very popular idea so far anyway.
After each round, should teams be chosen again and switched with different number of team members? If we do end up doing multiple rounds, that would be good. It would be a good way to mix up perspectives, and get cardsmiths a chance to interact with others that they haven't interacted with before.
How many people should be in each team? 4-5 would be best. As @ningyounk pointed out, it's small enough that everyone's voice should be heard, while large enough that if 1 or 2 people end up preoccupied with something in their personal life, that still leaves 3-4 people to work on the set. However, the number of possible teams is another thing to keep in mind. Let's say the aim is to have at least 3 teams. If each team must consist of a minimum of 4 cardsmiths, then you'd need a minimum of 12 people to sign up. So, so far you're good. If you want more than 3 teams, though, you'll either need more people to sign up or a riskier minimum of 3 people per team.
Since I'm not a loner and I want to join to, can I join and have the winner be voted (because entries will be few)? Sure! I'm fine with each team voting on the others.
Should I have all cards made in 1 set? As in, 1 account, with 1 set for every team? I think that would be the best way to handle it.
Should all the cards just be posted separately (I wouldn't suggest that)? Nah, that'd be a mess.
Should we have a premium member in each team so all cards they decide on will be put in that set (They could be team captains, but I don't think there are enough premium members)? Sure, if enough premium members sign up!
Should I get a good-hearted person like Corwinnn to make a set for each team? What exactly do you mean by that? Are you suggesting that Corwinnn choose the basic concept for each set that each team will work on? If so, a) That would avoid the problem of multiple people not being able to decide on one thing, or two teams having overly similar concepts, but on the other hand, b) That would restrict the freedom/creativity of the teams.
Can the winner be chosen by popular vote? Well, unless you ask someone else to judge in your place, if you're participating as a contestant, then yeah!

I feel honored being inspiration for this contest and being suggested as a team leader. To be true, that's a really responsible job and I am not sure, I can fulfill it. I'd rather like being a normal contestant.

I HAVE CHANGED THE LENGTH OF THE CONTEST TO NEXT WEDNESDAY (HOPEFULLY), WHICH WILL MAKE 1 WEEK.
